Important Notice	All memberships are based on a calendar year and carry a full year commitment. Any member wishing to resign must submit that request in writing to the Board of Directors. The Board of Directors may determine that a resigning member is responsible for the balance of dues for the full year. Refer to the club's bylaws for further information.
	I hereby make application for membership in the Beaver Hills Country Club. I agree, if elected, to read carefully and abide by the bylaws, rules, and policies of the club now in force and/or hereafter enacted during the term of my membership. My check for initiation, if applicable, is attached. Any past due account will accrue interest at 18% per year (1.5% per month) until paid in full.
